Personalized Care Plan Assessment

As part of our assessment process, both for new move-ins and at least twice annually, we evaluate the health and abilities of each Resident. This ensures we can offer the best care catered to exactly what each one needs. 

Here, you can see some of the functions we evaluate with you, and can talk about as part of your tour to be sure you know and we know how to provide world-class care, dignity, safety, and engagement. 

Please, ask us questions about our evaluation process, and any other specific concerns you might have. We’ll review:

  • Bathing and hygiene support needed
  • Dressing and grooming help and frequency of help
  • Oxygen use, CPAP, and other equipment
  • Sensory & communication impairment 
  • Bowel & bladder care and support
  • Behavioral assessments 
  • Escort and reminder help including meal support 
  • Medication services and other medical care
  • Transfers needed
  • Diabetic management

In addition to our assessment for a Plan of Care, Meadow Peak also offers a variety of fun activities, community engagements, and premium, quality-of-life services and accommodations for residents. Some are included as a resident and some are available for reasonable additional charges. A list can be provided as part of your tour and planning packet as you consider Meadow Peak.

Meadow Peak Assisted Living & Memory Care pricing works with our assessed needed level of care. Consequently, so does our pricing. As we meet and discuss levels of care and other optional accommodations, we’ll create an individualized costing structure and help you understand what support and coverage might be available to you.
